Junior Year Parent Questionnaire

Each junior's parents need to complete this questionnaire before we begin our college advising sessions. Plan at least 30 minutes to get through the questionnaire. Your completed questionnaire will be sent to Rebecca Lilienstern, Director of College Advising. Once she receives completed forms from you and your child, College Advising will contact you to schedule your junior family meeting so that we can work toward building a list of colleges to explore. We are here to help your child find the colleges that are the best fit.

Please respond to each question, even if your answer is "no," or "not applicable."

Your letter will add a personal touch to our description of your child’s personality and character and will be immensely helpful to us.

To help write the letter, you may wish to choose one or more of the following questions:

  • Choose two or three adjectives to describe your child and give one specific anecdote to support each adjective.
  • Assess your child’s strengths and weaknesses.
  • Write a letter to your child’s kindergarten teacher in which you describe all the changes that she or he has experienced over the years.
