Junior Year Student Questionnaire

Each junior needs to complete this questionnaire before we begin our college advising sessions. Plan at least 30 minutes to get through the questionnaire. Your completed questionnaire will be sent to Rebecca Lilienstern, Director of College Advising. Once she receives completed forms from you and your parents, College Advising will contact you to schedule your junior family meeting.

These questions are designed to help USJ's College Advising Office know you better so that we may guide you as you begin to build a list of colleges to explore. We are here to help you find the colleges that are the best fit for you.

18. List any athletic, artistic or musical awards that you have received in the last three years.
