If you are a senior, you need to fill out the senior release form so USJ can send your grades, test scores, etc. to the colleges to which you are applying. USJ graduates who need their high school transcript sent to another institution can download the transcript release form, below. Please allow one to two weeks for the processing of transcripts.

Juniors and their parents need to each complete an online questionnaire before their first meeting with college advising is scheduled.

Plan at least 30 minutes to complete each form. Once you submit the form, it is sent to Rebecca Lilienstern, Director of College Advising. Once both forms are completed, College Advising will contact you to set up your family meeting.

Support USJ and leave a lasting legacy by purchasing a brick to be used to line the walkway to the front entrance to the Upper School.

Here is the form for mixed carpools in the Lower School. You do not need to fill it out if you already submitted the form at the end of last school year. If your child is not in a mixed carpool, he or she will get a carpool tag from his or her teacher.
