Sports Physicals	Field Trips
The TSSAA requires that parents/medical personnel fill out several forms so that a child can play a sport. Sports Physical Form	This parent permission form is designed to work for most USJ, off campus field trips. Field Trip Permission Form

Cafeteria Debit Account	Kroger Card
Use this form the first time you put money on your child’s cafeteria debit card. Cafeteria Debit Form	When you shop at Kroger with your USJ Kroger card, 5 percent of your purchase cost is given to USJ. Fill out the attached form to get a Kroger card. Kroger Card Form

College Advising	Mixed Carpool
As juniors and seniors prepare for their first college advising meeting, they need to bring two completed forms to their meeting. One form is filled out by parents; the second is filled out by the student. You can download the forms in pdf format here. You do not need to fill it out if you already submitted the form at the end of last school year. If your child is not in a mixed carpool, he or she will get a carpool tag from his or her teacher. PAL League	Brick Legacy
This is the registration form for your child to play in the basketball PAL league. PAL League Registration Form	Support USJ and leave a lasting legacy by purchasing a brick to be used to line the walkway to the front entrance to the Upper School. Brick order form
